Position Title: Wound Care Nurse - Local Travel

Position Overview:
MLee Healthcare has partnered with a state recognized home healthcare provider who is rapidly expanding their Austin market. We are looking to establish a team of motivated, and qualified nurses to respond within 24 to 48 hours of a patient referral. The RN will be responsible for providing high-quality nursing care to patients in their homes. The role includes patient admission, assessment, care planning, and ongoing follow-up visits to ensure optimal patient outcomes.
Key Responsibilities:
- Patient Admission and Assessment:
- Conduct initial patient assessments and health evaluations.
- Develop and implement individualized care plans based on patient needs.
- Educate patients and families about care plans and health management.
- Care Coordination:
- Collaborate with multidisciplinary healthcare team members to ensure continuity of care.
- Coordinate and communicate with physicians, therapists, and other healthcare providers.
- Adjust care plans as necessary based on patient progress and feedback from the healthcare team.
- Patient Care and Management:
- Provide direct nursing care, including medication administration, wound care, and other skilled nursing tasks.
- Monitor and document patient health status and progress.
- Ensure patient safety and comfort at all times.
- Weekly Follow-Ups:
- Conduct weekly follow-up visits to assess patient progress.
- Update care plans and interventions as needed.
- Provide ongoing education and support to patients and families.
- Compliance and Documentation:
- Maintain accurate and timely patient records in compliance with healthcare regulations.
- Ensure adherence to all healthcare policies and procedures.
- Participate in quality improvement initiatives and audits.
Qualifications:
- Education:
- Associate or Bachelor’s Degree in Nursing from an accredited program.
- Licensure/Certification:
- Current and valid RN license in the state of practice.
- BLS certification required; ACLS and other certifications preferred.
- Experience:
- Minimum of 2 years of nursing experience, with a preference for home healthcare or hospice experience.
- Strong clinical skills in patient assessment, care planning, and management.
- Skills:
- Excellent communication and interpersonal skills.
- Ability to work independently and manage multiple patients.
- Proficiency in electronic medical records (EMR) system
