Position Title: Wound Care Nurse - Local Travel

Position Overview:

MLee Healthcare has partnered with a state recognized home healthcare provider who is rapidly expanding their Austin market. We are looking to establish a team of motivated, and qualified nurses to respond within 24 to 48 hours of a patient referral. The RN will be responsible for providing high-quality nursing care to patients in their homes. The role includes patient admission, assessment, care planning, and ongoing follow-up visits to ensure optimal patient outcomes.

Key Responsibilities:

  • Patient Admission and Assessment:
    • Conduct initial patient assessments and health evaluations.
    • Develop and implement individualized care plans based on patient needs.
    • Educate patients and families about care plans and health management.
  • Care Coordination:
    • Collaborate with multidisciplinary healthcare team members to ensure continuity of care.
    • Coordinate and communicate with physicians, therapists, and other healthcare providers.
    • Adjust care plans as necessary based on patient progress and feedback from the healthcare team.
  • Patient Care and Management:
    • Provide direct nursing care, including medication administration, wound care, and other skilled nursing tasks.
    • Monitor and document patient health status and progress.
    • Ensure patient safety and comfort at all times.
  • Weekly Follow-Ups:
    • Conduct weekly follow-up visits to assess patient progress.
    • Update care plans and interventions as needed.
    • Provide ongoing education and support to patients and families.
  • Compliance and Documentation:
    • Maintain accurate and timely patient records in compliance with healthcare regulations.
    • Ensure adherence to all healthcare policies and procedures.
    • Participate in quality improvement initiatives and audits.

Qualifications:

  • Education:
    • Associate or Bachelor’s Degree in Nursing from an accredited program.
  • Licensure/Certification:
    • Current and valid RN license in the state of practice.
    • BLS certification required; ACLS and other certifications preferred.
  • Experience:
    • Minimum of 2 years of nursing experience, with a preference for home healthcare or hospice experience.
    • Strong clinical skills in patient assessment, care planning, and management.
  • Skills:
    • Excellent communication and interpersonal skills.
    • Ability to work independently and manage multiple patients.
    • Proficiency in electronic medical records (EMR) system
