As a Clinical Pharmacist, you will play a crucial role in optimizing medication therapy management and ensuring the highest standards of pharmaceutical care. Your responsibilities will include:

• Conducting patient assessments to determine medication therapy needs.
• Collaborating with healthcare providers to develop and implement patient-specific treatment plans.
• Monitoring and adjusting medication regimens to achieve therapeutic goals.
• Providing drug information and education to patients and healthcare professionals.
• Participating in interdisciplinary rounds and healthcare team meetings.

Qualifications: To be successful in this role, you should possess the following qualifications:

• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.
• Current Texas state pharmacy license in good standing.
• Board Certification in Pharmacotherapy (BCPS) or other relevant certifications preferred.
• Strong clinical knowledge and understanding of evidence-based medicine.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a team-oriented environment.

Experience:

• Minimum of one year of clinical pharmacy experience.
• Experience in an acute care or hospital setting is preferred.
