MLR (MLee Recruiting) is excited to offer a full-time, permanent position for a Physical Therapist - Home Health to join a vibrant team at a well-respected acute-care hospital located near Conway, South Carolina!

Compensation: Competitive salary plus a $15,000 sign-on bonus.

Key Responsibilities:

• Patient Evaluations: Conduct thorough assessments of patients in their home settings to identify their physical capabilities, limitations, and rehabilitation needs.
• Work Schedule: 80 hours biweekly, Monday through Friday.
• Personalized Treatment Plans: Develop and implement tailored therapy programs based on patient evaluations, medical history, and physician recommendations.
• In-Home Therapy Services: Deliver physical therapy services directly in patients’ homes, which may include exercise routines, manual therapy, and mobility enhancement strategies.
• Patient and Family Education: Teach patients and their families about exercise techniques, therapeutic approaches, and lifestyle modifications to support recovery and improve functional abilities.
• Progress Monitoring and Plan Adjustments: Regularly track and document patient progress, adjusting treatment plans as needed to ensure optimal outcomes.
• Collaborative Care: Collaborate with physicians, healthcare providers, and caregivers to provide coordinated and effective patient care.
• Emergency Response: Address urgent situations or emergencies that may occur during home visits, delivering appropriate care and interventions as necessary.
