Physical Therapist - Home Health

 

MLR (MLee Recruiting) is actively seeking a full-time, permanent Physical Therapist for a direct-hire role with a supportive home health team at a well-established acute-care hospital near Conway, South Carolina.

 

Compensation: Offers a competitive salary along with a $15,000 sign-on bonus.

 

Schedule: 80 hours bi-weekly, Monday through Friday.

 

 

Physical Therapist Duties

  • Patient Assessments: Perform comprehensive evaluations of patients in their home environments to determine physical capabilities, limitations, and rehabilitation goals.
  • Customized Therapy Plans: Design and implement tailored therapy programs based on evaluations, patient history, and physician referrals.
  • Home-Based Therapy: Provide physical therapy directly at patients’ homes, using exercise routines, manual techniques, and other approaches to improve mobility and functionality.
  • Patient and Family Instruction: Educate patients and their families on exercises, therapy techniques, and lifestyle changes to support recovery and improve quality of life.
  • Progress Monitoring: Track patient progress frequently and adjust treatment plans as needed to achieve optimal results.
  • Coordinated Care: Collaborate closely with physicians, healthcare teams, and caregivers to ensure well-rounded, cohesive care.
  • Emergency Response: Manage urgent issues that may arise during home visits and provide necessary care promptly.

Requirements

  • Driver’s License & Insurance: Must possess a valid South Carolina driver’s license and auto insurance; candidates must be 21 or older to comply with fleet policy requirements.
  • Licensure & Certification: A current Physical Therapy license and Basic Life Support (BLS) certification are necessary. Professional liability insurance is recommended but not required.
  • Regulatory Compliance: Keep all necessary licenses and registrations current as per state and organizational guidelines.
