Home Health / Hospice Nurse Job Description 

• Receive patient assignments from the Operations Leader or Clinical Resource Team Leader.
• Communicate effectively about patient care, business operations, and foster a positive work environment.
• Assume responsibility for patient and family care, including assessment, planning, implementation, and evaluation.
• Initiate and maintain communication with attending physicians, hospice staff, and other agencies to ensure optimal patient care and resource utilization.
• Collaborate regularly with the Operations Leader or designated personnel to review care strategies.
• Maintain consistent communication with attending physicians regarding patient and family care.
• Collect data on physical, psychological, social, and spiritual factors influencing patient and family health, and integrate this information into the care plan.

Home Health / Hospice Nurse Certifications

• Minimum of two years of nursing experience preferred; one year in home care is highly desirable.
• Current BLS Certification.
• Valid Registered Nurse licensure in the state of South Carolina.
• Valid driver’s license and proof of vehicle insurance.
