Each morning, you’ll begin not in a hospital hallway, but on the road—traveling from home to home, helping patients regain their footing in the places they love most. These are people recovering from surgery, injury, illness, or chronic conditions—and their greatest desire is often the simplest one: to move again with confidence, dignity, and autonomy.

 

In this role, you’ll conduct in-home evaluations, assess mobility and strength, and design personalized physical therapy plans aligned with physician guidance. You’ll direct treatment, educate families and caregivers, and provide insight on therapeutic equipment and assistive devices. Whether you’re retraining gait patterns, refining range of motion, or coaching someone through stairs for the first time post-injury, your impact will be immediate and profound.

 

You’ll document clearly and efficiently, communicate fluidly with the full care team, and guide other home health professionals toward a unified plan of care. You’ll also supervise Physical Therapy Assistants with purpose and mentorship—ensuring high standards, consistency, and compassion in every visit, even when you’re not the one making it.

What We’re Looking For

 

To thrive in this role, you’ll need a strong foundation in clinical skill, a deep respect for patient-centered care, and the adaptability to meet each client where they are—literally and emotionally.

 

You’ll need a Bachelor’s degree (or higher) in Physical Therapy from an accredited program, along with an active Ohio PT license and current CPR certification. A valid driver’s license and proof of auto insurance are required, as you’ll be traveling to patients’ homes each day.

 

Experience in home health or community-based rehab is preferred, but we welcome strong clinicians from all settings who bring compassion, integrity, and curiosity to their work.

 

You should be confident with functional evaluations, plan-of-care development, documentation systems (including OASIS), and interdisciplinary communication. You should also be comfortable supervising Physical Therapy Assistants and coordinating with RNs, aides, and case managers for seamless patient outcomes.
