Description

• Conduct evaluations for patients in both inpatient and outpatient settings, tailoring individualized treatment plans for optimal recovery.
• Deliver specialized physical therapy interventions designed to enhance mobility, strength, balance, and overall function.
• Engage collaboratively with nursing staff, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes.
• Inform patients and their families about therapeutic objectives, discharge strategies, and home exercise routines.
• Maintain thorough documentation of evaluations, treatments, and progress according to facility and regulatory standards.
• Contribute to interdisciplinary team meetings and rounds to ensure cohesive patient care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited program.

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory.

Skills

• Previous experience is highly preferred.
