Join our team as a Clinical Pharmacist at near Yonkers, NY, reporting directly to the Director of Pharmacy Services. As a Clinical Pharmacist, you'll oversee the rational antibiotic therapy within our hospital, focusing on critical care or high-risk patients.

Responsibilities:

• Assess medication orders and prescriptions for appropriateness, safety, and efficacy
• Collaborate with healthcare providers to optimize medication therapy outcomes
• Monitor patients for adverse drug reactions and medication-related problems
• Maintain accurate and complete clinical documentation
• Provide patient education on medication use, adherence, and potential side effects
• Participate in quality improvement initiatives related to medication management
• Work closely with interdisciplinary healthcare teams

Requirements:

• PharmD degree from an accredited school of Pharmacy
• Current New York State Registration
• Completion of a pharmacy residency program, preferably in clinical pharmacy or a related specialty
