As a Case Manager, you will work closely with physicians, nurses, and other healthcare professionals to coordinate high-quality, cost-effective care for patients throughout their hospital stay and discharge process.
Whether you are a Registered Nurse (RN) or a licensed Social Worker (LMSW, LISW, or LCSW), this opportunity offers a chance to make a lasting impact while living in one of the most culturally rich and naturally beautiful regions of the Southwest.
What You’ll Do:
- Conduct comprehensive assessments to identify patient needs and develop individualized discharge plans
- Coordinate services including home health, skilled nursing, rehabilitation, and community-based resources
- Collaborate across disciplines to ensure optimal patient outcomes and safe transitions of care
- Advocate for patients and their families by navigating complex healthcare systems and insurance processes
- Support efforts to reduce length of stay (LOS), readmission rates, and resource utilization
- Provide timely and thorough documentation in EMR systems according to regulatory requirements
- Educate patients and families about available services and follow-up care
- Issue required Medicare notifications and assist with appeals when needed
Qualifications:
Education:
- RN: Graduate of accredited nursing program (BSN preferred)
- OR MSW: Master's in Social Work from an accredited program
Licensure & Certification:
- RN: Active RN license in New Mexico
- Social Work: LMSW, LISW, or LCSW in New Mexico
- BLS required
- Case management certifications (CCM, ACM, or RN-BC) preferred
Experience:
- At least 2 years of healthcare experience, preferably 1 year in an acute care or hospital setting
Skills:
- Effective communicator and strong interpersonal skills
- Detail-oriented with strong problem-solving ability
- Comfortable navigating EMR systems and Microsoft Office
- Knowledge of insurance regulations and post-acute care options
