Key Responsibilities
- Collaborate with physicians, nursing, social workers, and support staff to facilitate efficient and safe patient throughput
- Conduct comprehensive assessments to identify clinical, psychosocial, and post-acute care needs
- Develop individualized discharge plans, including coordination of skilled nursing, rehabilitation, home health, hospice, and durable medical equipment (DME)
- Serve as a liaison between patients, families, and community resources, ensuring continuity of care
- Advocate for patient rights while balancing utilization review and cost-effective care practices
- Monitor length of stay (LOS), readmission risk, and quality metrics
- Maintain documentation in compliance with regulatory standards and hospital policy
- Communicate clearly with payers to resolve insurance issues and support appeals
- Assist in regulatory tasks such as issuing Medicare Important Messages and Non-Coverage Notices
Requirements
Education:
- RN: Graduate of an accredited nursing program (BSN preferred)
- OR Social Work: Masters in social work (MSW) from an accredited institution
Licensure & Certifications:
- RN: Active RN license in New Mexico
- Social Work: LMSW, LISW, or LCSW in New Mexico
- Case Management certification (ACM, CCM, or RN-BC) preferred
- BLS certification required
Experience:
- Minimum of 2 years in healthcare, with 1+ year in an acute care setting preferred
- Familiarity with discharge planning, medical necessity criteria, and community resources
Skills:
- Strong communication, critical thinking, and time-management skills
- Ability to multitask and adapt in a fast-paced, patient-first environment
- Proficient in EMR and Microsoft Office platforms
