Schedule: Day Shift | Full-Time Hours

Whether you're a Registered Nurse (RN) or a Licensed Master Social Worker (LMSW, LISW, or LCSW), this role empowers you to lead patient care coordination, enhance discharge planning, and support safe, efficient transitions across the care continuum.

Key Responsibilities: 

  • Conduct clinical and psychosocial assessments to determine discharge and care coordination needs 
  • Develop and execute personalized care plans that optimize outcomes and reduce readmissions 
  • Serve as a bridge between care teams, patients, families, and external providers 
  • Manage utilization review and payer communications to ensure coverage and compliance 
  • Facilitate referrals to post-acute care services including SNFs, rehab, home health, and hospice 
  • Educate patients and caregivers on resources, follow-up care, and wellness planning 
  • Ensure complete, timely documentation in the electronic medical record 

 

Qualifications: 

Education & Licensure: 

  • RN: Graduate of an accredited nursing program; BSN preferred 
  • LMSW/LISW/LCSW: Master’s in Social Work from an accredited institution 
  • Active RN or LMSW/LISW/LCSW license in the state of New Mexico 
  • BLS certification required 
  • Preferred certifications: ACM, CCM, RN-BC in Case Management 

Experience: 

  • Minimum of 2 years in healthcare, preferably with acute care or hospital case management experience 

Skills: 

  • Exceptional communication and organizational skills 
  • Proficient in discharge planning, resource coordination, and clinical documentation 
  • Familiarity with payer regulations and EMR/case management software 
  • Able to work independently while collaborating with an interdisciplinary team
