This full-time position is open to experienced Registered Nurses (RN) and Licensed Master Social Workers (LMSW, LISW, LCSW) looking to coordinate and advocate for patients throughout their care journey.

What You’ll Do:
- Conduct assessments to determine patients' care needs and develop safe discharge plans
- Coordinate with post-acute care providers including home health, rehab, and skilled nursing facilities
- Collaborate across departments to promote quality care and timely transitions
- Act as a central liaison for families, healthcare teams, insurance providers, and community resources
- Ensure compliance with care coordination standards, documentation, and patient education
What We’re Looking For:
Education & Licensure:
- RN: Associate or Bachelor’s Degree in Nursing from an accredited program
- LMSW/LISW/LCSW: Master’s Degree in Social Work
- Active RN or Social Work license in New Mexico required
- BLS certification required
- ACM, CCM, or RN-BC case management certification preferred
Experience & Skills:
- Minimum of 2 years in healthcare, ideally in an acute care setting
- Proficient in discharge planning, case documentation, and regulatory compliance
- Strong communication and critical thinking abilities
- EMR experience and familiarity with case management software preferred
