Clinical Pharmacist
We are looking for a skilled and committed Clinical Pharmacist to join our team at a respected medical center. In this role, you will be essential in delivering optimal pharmaceutical care to our patients while working closely with the healthcare team to ensure safe and effective medication management.
Responsibilities
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient-specific medication therapy plans.
- Conduct medication reviews, assess drug interactions, and recommend appropriate interventions to optimize medication therapy.
- Provide drug information and education to healthcare professionals, patients, and their families.
- Monitor patient response to medications, adjust doses, and provide guidance on potential side effects or adverse reactions.
- Participate in medication reconciliation processes and contribute to medication safety initiatives.
- Contribute to the development and implementation of evidence-based pharmacy protocols and guidelines.
- Ensure compliance with regulatory standards, policies, and procedures related to medication management.
- Contribute to the training and mentoring of pharmacy students and residents.
Qualifications
- Preferred: Doctor of Pharmacy (Pharm.D.) degree from an accredited college of pharmacy.
- Active pharmacist license in the respective state.
- Preferred minimum of 2 years of clinical pharmacy experience in a hospital or healthcare setting.
