Case Manager - Registered Nurse
We are seeking a skilled and compassionate Case Manager Registered Nurse (RN) to join our healthcare team in New Mexico. In this critical role, you will coordinate patient care, ensure continuity of services, and advocate for the best outcomes for individuals under your care. The ideal candidate will have a strong clinical background, exceptional communication skills, and a commitment to improving the health and well-being of patients through personalized case management.
Key Responsibilities:
- Conduct thorough patient assessments to determine medical, emotional, and social needs.
- Develop and implement individualized care plans to ensure optimal patient outcomes.
- Coordinate care between patients, healthcare providers, and family members to ensure comprehensive and continuous care.
- Monitor patient progress, adjust care plans as needed, and ensure adherence to treatment protocols.
- Educate patients and families on managing chronic conditions, medications, and lifestyle modifications.
- Advocate for patients to ensure they receive the appropriate services and resources within the healthcare system.
- Provide case management services including discharge planning, follow-up care, and referrals to community resources.
- Maintain accurate and up-to-date patient records in compliance with privacy and healthcare regulations.
- Collaborate with multidisciplinary teams, including physicians, social workers, and other healthcare professionals.
- Participate in quality improvement initiatives and contribute to the development of best practices in care management.
Qualifications:
- Active Registered Nurse (RN) license in the state of New Mexico.
- Minimum of 2 years of clinical nursing experience (hospital or community-based nursing preferred).
- Previous experience in case management or care coordination is strongly preferred.
- Strong knowledge of healthcare services, community resources, and patient advocacy.
- Exceptional communication and interpersonal skills.
- Ability to assess and develop comprehensive care plans.
- Proficient in using electronic health records (EHR) systems and other healthcare software.
- Bilingual (Spanish-English) is a plus.
- Reliable transportation and ability to travel within the assigned region.
