RN – Public Health Nurse / Home Visits

Qualifications:
- Current RN license in the state of Iowa
- Minimum of one year of clinical nursing experience (home health or public health preferred)
- Valid driver’s license and reliable transportation
- BLS certification (American Heart Association)
- Bachelor's degree preferred, or two years of related work experience
- Ability to work independently and manage time effectively
- Excellent assessment, communication, and documentation skills
- Must have access to a personal cell phone and be comfortable using EMRs
What You Will Do:
- Conduct in-home nursing assessments and provide direct patient care
- Develop, implement, and evaluate individualized care plans
- Administer medications, manage treatments, and provide health education
- Supervise Home Health Aides in the home environment
- Collaborate with interdisciplinary teams to coordinate services
- Provide community health education and public health outreach as needed
- Participate in on-call rotation (typically 1–2 weekends per month)
- Accurately document all care using electronic medical records
