Pharmacist - Hospital Inpatient Setting

Position Qualifications:

• Doctor of Pharmacy (Pharm.D.) degree from an accredited institution
• Valid Montana state pharmacy license
• Experience in an inpatient or hospital pharmacy setting is preferred
• Strong communication and critical thinking skills
• Ability to work collaboratively in a fast-paced, multidisciplinary team environment
• Commitment to providing high-quality, patient-centered care

Responsibilities:

• Provide clinical pharmacy services to hospitalized patients, ensuring safe and effective use of medications.
• Review and verify medication orders, ensuring appropriate drug selection, dosage, and administration.
• Collaborate with healthcare teams to optimize medication therapies and patient care plans.
• Monitor patient drug therapies for efficacy, side effects, and drug interactions.
• Provide patient education on medications, ensuring understanding and adherence to prescribed regimens.
• Participate daily rounds with physicians, nurses, and other healthcare providers to offer medication-related advice and recommendations.
• Maintain accurate medication records in compliance with hospital policies and regulatory guidelines.
