Description

• Conduct evaluations and assessments for patients in inpatient or outpatient settings to develop tailored treatment plans
• Deliver physical therapy interventions that prioritize mobility, strength, balance, and functional improvement
• Engage in collaborative practices with nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes
• Educate patients and their families on therapeutic goals, discharge plans, and at-home exercise regimens
• Document patient evaluations, treatments delivered, and progress updates following facility and regulatory standards
• Participate actively in interdisciplinary conferences and care team rounds to ensure seamless care transitions

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory
