Your Responsibilities Include

• Performing thorough patient evaluations, coordinating diagnostic testing, and crafting personalized care strategies
• Diagnosing and managing acute, chronic, and episodic health conditions in collaboration with healthcare teams or autonomously (depending on your credentials)
• Informing patients and families about prevention, wellness, and effective disease management solutions
• Prescribing necessary medications and treatments within the scope of practice
• Engaging in patient follow-up and comprehensive chronic illness management
• Working in tandem with interdisciplinary teams to assure integrated, holistic care
• Accurately documenting patient interactions and care within electronic medical record systems (EMR)

Education Requirements

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Successful completion of an accredited Nurse Practitioner program

Required Certifications

• Active Nurse Practitioner license in Minnesota
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or an equivalent specialty certification
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the ability to acquire these within 90 days)
