Requirements:

- Pharmacy degree from a fifth-year college or university program equivalent
- Indiana Pharmacy License
- Preferred: Recent experience as a hospital pharmacist within the last 5 years. New graduates with robust hospital work histories will be considered.
- Preferred: PharmD and/or PGY-1 Residency
- Will accept retail pharmacist experience

Education:
Minimum Level of Education Required: Doctorate degree
Preferred educational qualifications: Pharmacy Residency
