We are currently seeking a Clinical Pharmacist to join our growing team of healthcare professionals.

 

Position Overview:

 

As a Clinical Pharmacist, you will collaborate closely with doctors, nurses, and other healthcare providers to optimize medication therapy and ensure the best possible outcomes for our patients. You will also play a key role in patient education, drug utilization review, and medication management across various clinical settings, including inpatient and outpatient care.

 

Key Responsibilities:

  • Medication Management: Review and manage patient medication therapies, adjusting dosages and regimens as necessary for optimal therapeutic outcomes.
  • Collaborate with Healthcare Teams: Work closely with physicians, nurses, and other healthcare professionals to provide individualized patient care and ensure safe, effective use of medications.
  • Patient Education: Counsel patients and their families on proper medication use, side effects, interactions, and lifestyle modifications to improve medication adherence.
  • Clinical Decision Support: Provide expert drug information and therapeutic recommendations to optimize patient care in critical care, acute care, and other specialized departments.
  • Medication Therapy Reviews: Monitor and evaluate the effectiveness of prescribed treatments, ensuring appropriate therapeutic interventions.
  • Clinical Research: Contribute to research and clinical trials related to pharmacotherapy and medication management.
  • Compliance and Safety: Ensure all medications are administered in compliance with hospital policies, regulatory guidelines, and safety protocols.

Qualifications:

  • Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school.
  • Licensure: Active pharmacist license in the state of Indiana or eligibility to obtain licensure upon hire.
  • Experience: Prior clinical pharmacy experience in hospital or healthcare settings preferred. New graduates with clinical rotations are also encouraged to apply.
  • Certifications: Board certification in pharmacotherapy (BCPS) or other clinical pharmacy credentials is a plus but not required.
  • Skills: Strong clinical knowledge, excellent communication skills, and the ability to work in a fast-paced, multidisciplinary team environment.
