Position Highlights:

  • Flexible Scheduling: Enjoy a balanced work-life rhythm with a 9 AM - 5 PM schedule and rotational weekend calls every 6-8 weeks.
  • Patient-Focused Role: Serve as a guiding light of comfort and support for patients and their families during their hospice journey.
  • Collaborative Care: Work alongside a supportive interdisciplinary team to enhance patient care and optimize resources.

Experience and Qualifications:

  • Licensure: Must hold an Illinois RN license; a BSN is a plus.
  • Experience: Minimum of one year of recent acute care or hospice experience, with a strong commitment to the hospice philosophy of care.

Your Role:

As a valued member of our Hospice Care team, you’ll receive patient assignments from our Hospice Operations Leader. Your mission is to foster a nurturing environment through effective communication while performing essential duties, including:

  • Patient Care Management: Assess, plan, implement, and evaluate care to ensure the highest quality for our patients.
  • Team Collaboration: Collaborate closely with attending physicians, hospice staff, and other agencies to coordinate optimal care.
  • Communication: Regularly update and engage with our Hospice Operations Leader and attending physicians to enhance patient care.

Your Responsibilities Will Include:

  • Detailed Record-Keeping: Maintain comprehensive and accurate patient records to ensure clarity in the care process.
  • Commitment to Hospice Philosophy: Demonstrate unwavering dedication to providing compassionate care, supported by your experience.

Requirements:

  • Current RN Licensure: Must be licensed in Illinois.
  • BSN Preferred: A Bachelor’s degree in Nursing is advantageous.
  • Auto Insurance: Proof of auto insurance covering bodily injury and property damage.
