Position Highlights:

Experience and Qualifications:

  • Licensure: Must hold an Illinois RN license. A BSN is a plus.
  • Experience: At least one year of recent acute care or hospice experience, with a strong commitment to the hospice philosophy of care.

Your Role:

As a cherished member of our Hospice Care team, you’ll receive patient assignments from our Hospice Operations Leader. Your mission will be to create a nurturing environment through effective communication while performing essential duties, including:

  • Patient Care Management: Assess, plan, implement, and evaluate patient care to ensure the highest quality.
  • Team Collaboration: Work closely with attending physicians, hospice staff, and other agencies to coordinate optimal care.
  • Communication: Regularly update and collaborate with our Hospice Operations Leader and attending physicians to review and enhance patient care.

Your Responsibilities Will Include:

  • Detailed Record-Keeping: Maintain comprehensive and accurate patient records to ensure clarity in the care process.
  • Commitment to Hospice Philosophy: Demonstrate unwavering dedication to providing compassionate care, supported by your experience.

Requirements:

  • Current RN Licensure: Must be licensed in Illinois.
  • BSN Preferred: A Bachelor’s degree in Nursing is advantageous.
  • Auto Insurance: Proof of auto insurance covering bodily injury and property damage.
