Key Responsibilities:

  • Patient Care: Review and monitor patient medication regimens to ensure appropriate therapeutic outcomes. Work with the healthcare team to develop and adjust medication therapies.
  • Consultation: Provide medication counseling to patients and healthcare providers, ensuring safe and effective use of medications.
  • Medication Management: Oversee drug utilization reviews, formulary management, and medication safety practices across the institution.
  • Education: Educate patients and healthcare teams on the proper use of medications, potential side effects, and new treatment protocols.
  • Clinical Support: Assist in the development and implementation of clinical guidelines, protocols, and policies to improve patient care and safety.

Qualifications:

  • Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Licensure: Active Illinois Pharmacist license in good standing (or eligibility for licensure).
  • Experience: Clinical pharmacy experience preferred, particularly in hospital or outpatient settings.
  • Skills: Strong communication and interpersonal skills, with a deep understanding of pharmacology, medication therapy management, and clinical practices.
