Home Health Nurse – RN / Public Health Registered Nurse

Looking to bring your nursing skills out into the community? As a Home Health Nurse and Public Health RN, you’ll provide meaningful, one-on-one care in patients’ homes throughout Central Iowa. This role gives you the independence to manage your day while being part of a supportive, mission-driven team that truly puts patients first.

Qualifications:
- Active Iowa RN license
- Minimum of one year clinical experience (home health or public health preferred)
- Current BLS certification
- Valid driver’s license, dependable transportation, and auto insurance
- Bachelor’s degree preferred or equivalent nursing background
- Strong documentation, assessment, and communication skills
- Self-starter who enjoys working independently
- Must have a personal cell phone
What You Will Do:
- Provide compassionate nursing care in home settings across a designated area
- Perform assessments, administer treatments, and educate patients and caregivers
- Develop care plans and monitor patient progress
- Coordinate care with interdisciplinary teams and community health partners
- Supervise Home Health Aides in the field
- Participate in local health initiatives and public education programs
- Be part of an on-call rotation (usually 1–2 weekends per month)
- Use an electronic health record system for thorough documentation
