Description

• Conduct evaluations for patients in either inpatient or outpatient settings and create tailored treatment plans to support their recovery journey
• Implement physical therapy interventions centered on enhancing mobility, strength, balance, and functional abilities
• Work collaboratively with a diverse team of nurses, physicians, case managers, and rehabilitation professionals to ensure optimal patient outcomes
• Engage patients and their families in therapeutic goals, discharge planning, and an effective home exercise program
• Document patient evaluations, treatments, and progress in alignment with facility and regulatory protocols
• Participate in care team rounds and interdisciplinary discussions to facilitate seamless care coordination

Education

• Doctorate in Physical Therapy (DPT) or a valid equivalent from an accredited institution

Certifications

• Licensed as a Physical Therapist in the state
• CPR certification strongly preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous, but not mandatory
