Licensed Practical Nurse– Home Health (full-time) in the Torrington, CT area.

What the work involves:

• Cover Torrington and surrounding towns. Requires covering 1 weekend/month, 1 major and 1 minor holidays/year
• Work where every moment matters.
• Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
• Records daily progress in EMR.
• Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
• Communicates/conferences with primary nurse and other disciplines.
