Registered Oncology Pharmacist
Requirements:
• Graduate from an accredited school of Pharmacy with minimum of a BS in Pharmacy.
• NYS Pharmacist license and Current New York registration required.
• 2 – 3 years hospital pharmacy experience;
• Minimum three (3) of annual CE requirements must be in Chemotherapy/Oncology.
• Oncology Certification by NYSCHP or BPS or equivalent preferred.
• Experience in hematology oncology and infusion center operations preferred.
• Successful completion of Hospital and Departmental Orientation Program, according to hospital and Pharmacy policy and procedure

Responsibilities
• Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Gathers necessary data to promote patient safety by ensuring that each relevant medication is indicated, effective, safe, and that the patient is adherent to therapy Improves patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects
• Assesses relevant patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues as needed
• May design a drug therapy plan for patient-specific problems and persuasively justifies recommendations
