What You’ll Do:

  • Deliver Patient Care:
    • Conduct thorough physical exams and medical histories to assess symptoms and diagnose medical conditions.
    • Order and interpret diagnostic tests (blood work, imaging) to assist in treatment decisions.
    • Develop and implement individualized treatment plans tailored to each patient's unique needs.
    • Prescribe medications, therapies, and other interventions as part of comprehensive patient care.
    • Manage acute and chronic diseases, ensuring that all patient health conditions are addressed and monitored.
  • Patient Education and Support:
    • Educate patients and their families on managing health conditions, treatment options, and lifestyle changes.
    • Offer preventive care services, including health screenings, immunizations, and wellness check-ups.
    • Provide guidance on health maintenance, nutrition, and self-care strategies to ensure long-term health.
  • Collaborate with Healthcare Teams:
    • Collaborate with physicians, specialists, and healthcare professionals to coordinate care and develop treatment plans.
    • Participate in case discussions and provide insights on patient care management.
    • Offer support and guidance to nursing staff and other healthcare team members.
  • Monitor Patient Progress:
    • Regularly evaluate and monitor patients’ responses to treatments and make necessary adjustments to care plans.
    • Maintain accurate documentation of patient interactions, treatment plans, and progress in EHRs (electronic health records).
    • Advocate for improvements in care based on patient feedback and clinical assessments.
  • Ensure Compliance and Safety:
    • Adhere to all healthcare laws, regulations, and organizational policies.
    • Stay current with the latest medical developments, evidence-based practices, and industry standards.
    • Ensure patient confidentiality is upheld in accordance with HIPAA and privacy laws.

Qualifications:

  • Master’s Degree in Nursing from an accredited Nurse Practitioner program.
  • Active Nurse Practitioner (NP) License in Colorado.
  • Board Certification as a Nurse Practitioner (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, etc.).
  • BLS and ACLS certifications preferred.
  • Previous experience as a Nurse Practitioner or Registered Nurse (RN) is a plus.
  • Strong clinical skills for diagnosing, managing, and treating a range of health issues.
  • Excellent communication skills to engage with patients and collaborate with the healthcare team.
  • Ability to work independently and make informed clinical decisions.
  • Compassionate, patient-focused approach to care.
