What the work involves:

• We are recruiting for a mission-driven LCSW/LPC CCP - Integrated Behavioral Health to join our team!

• Provides high-quality clinical services to referred individuals, including short-term evidence-based interventions, support services, and collaboration with other teams as needed. (70%)
• Conducts brief diagnostic screening of patients using evidence-based assessments and screening tools. (5%)
• Provides health behavior change interventions using Motivational Interviewing. (5%)
• Consult with providers as part of the interdisciplinary team and generate recommendations, referrals, and case management as needed. (5%)
