Key Responsibilities:
- Medication Management: Review and manage patient medication plans, ensuring safe and effective use of pharmaceuticals.
- Clinical Consultations: Provide expert advice to healthcare professionals on medication therapies, including dosing, side effects, and drug interactions.
- Patient Education: Counsel patients on proper medication use, potential side effects, and adherence strategies.
- Interdisciplinary Collaboration: Work closely with physicians, nurses, and other healthcare team members to optimize patient treatment plans.
- Medication Reconciliation: Conduct thorough medication reviews during patient admissions, transfers, and discharges to prevent errors.
- Regulatory Compliance: Ensure compliance with all relevant pharmacy laws, regulations, and standards.
- Quality Improvement: Participate in initiatives aimed at improving medication safety and patient outcomes.
Qualifications:
- 1 or more years of Clinical Pharmacist experience is required.
- PharmD or RPh from an accredited school.
- Current and valid license to practice.
- Understanding of DEA and FDA health regulations.
- Responsible, detail-oriented person of upstanding integrity.
- Be able to handle a large workload while still working efficiently and accurately.
- High attention to detail and accuracy.
- Strong analytical and critical thinking skills.
- Personable and well-spoken with great customer service.
