Key Responsibilities:

  • Medication Management: Oversee patient medication plans to ensure safety and efficacy.
  • Expert Consultation: Collaborate with physicians, nurses, and other healthcare professionals on medication therapies, side effects, and drug interactions.
  • Patient Education: Provide patients with critical information on proper medication use and adherence strategies.
  • Interdisciplinary Collaboration: Partner with healthcare team members to optimize treatment plans.
  • Medication Reconciliation: Conduct thorough reviews during patient admissions, transfers, and discharges to prevent errors.
  • Regulatory Compliance: Ensure adherence to all pharmacy laws, regulations, and safety standards.

Qualifications:

  • PharmD or RPh from an accredited school.
  • Active and valid license to practice.
  • 1+ years of Clinical Pharmacist experience required.
  • Strong understanding of DEA and FDA health regulations.
  • Detail-oriented with a high level of accuracy and critical thinking skills.
