Key Responsibilities:
- Medication Management: Oversee patient medication plans to ensure safety and efficacy.
- Expert Consultation: Collaborate with physicians, nurses, and other healthcare professionals on medication therapies, side effects, and drug interactions.
- Patient Education: Provide patients with critical information on proper medication use and adherence strategies.
- Interdisciplinary Collaboration: Partner with healthcare team members to optimize treatment plans.
- Medication Reconciliation: Conduct thorough reviews during patient admissions, transfers, and discharges to prevent errors.
- Regulatory Compliance: Ensure adherence to all pharmacy laws, regulations, and safety standards.
Qualifications:
- PharmD or RPh from an accredited school.
- Active and valid license to practice.
- 1+ years of Clinical Pharmacist experience required.
- Strong understanding of DEA and FDA health regulations.
- Detail-oriented with a high level of accuracy and critical thinking skills.
