Key Responsibilities:
- Medication Management: Oversee and manage medication therapies to ensure patient safety, effectiveness, and optimal outcomes.
- Expert Consultation: Collaborate closely with physicians, nurses, and the healthcare team on medication therapies, addressing side effects, interactions, and treatment plans.
- Patient Education: Provide patients with essential information on proper medication use and strategies for adherence.
- Medication Reconciliation: Review and reconcile patient medications during admissions, transfers, and discharges to minimize errors.
- Regulatory Compliance: Ensure all pharmacy operations adhere to DEA, FDA, and local pharmacy regulations, maintaining the highest safety standards.
What We’re Looking For:
- Education: PharmD or RPh from an accredited school of pharmacy.
- Licensure: Active and valid license to practice as a Clinical Pharmacist in the state of New Mexico.
- Experience: 1+ years of experience in a clinical pharmacy role.
- Skills: Strong understanding of DEA and FDA regulations, critical thinking, attention to detail, and excellent collaboration skills.
