Clinical Pharmacist
We are currently seeking a knowledgeable and dedicated Clinical Pharmacist to join our team at a reputable medical center. As a Clinical Pharmacist, you will play a vital role in providing optimal pharmaceutical care to our patients and collaborating with the healthcare team to ensure safe and effective medication management.
Responsibilities
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient-specific medication therapy plans.
- Conduct medication reviews, assess drug interactions, and recommend appropriate interventions to optimize medication therapy.
- Provide drug information and education to healthcare professionals, patients, and their families.
- Monitor patient response to medications, adjust doses, and provide guidance on potential side effects or adverse reactions.
- Participate in medication reconciliation processes and contribute to medication safety initiatives.
- Contribute to the development and implementation of evidence-based pharmacy protocols and guidelines.
- Ensure compliance with regulatory standards, policies, and procedures related to medication management.
- Contribute to the training and mentoring of pharmacy students and residents.
Qualifications
- Preferred: Doctor of Pharmacy (Pharm.D.) degree from an accredited college of pharmacy.
- Active pharmacist license in the respective state.
- Preferred minimum of 2 years of clinical pharmacy experience in a hospital or healthcare setting.
