Job Description:

As a valued member of our Hospice Care team, you'll receive patient assignments from our Hospice Operations Leader. Your primary focus will be on fostering a positive work environment through effective verbal communication while assuming the critical responsibilities of assessing, planning, implementing, and evaluating patient care. Collaborative communication with attending physicians, hospice staff, and other agencies will be pivotal to ensuring optimal care and resource utilization. You'll maintain close communication with our Hospice Operations Leader for care reviews and uphold regular dialogue with attending physicians.

Your Responsibilities Will Include:

• Maintaining meticulous patient records to ensure clarity and accuracy in the care process.
• Demonstrating unwavering commitment to the hospice philosophy of care, supported by at least one year of recent acute care/hospice experience.

Requirements:

• Currently licensed as an RN in the state of Illinois.
• A BSN is preferred.
• Proof of auto insurance for bodily injury and property damage.
